Please keep it brief. The office will call you to schedule and go over the details. This form is not for emergencies or urgent symptoms.

Before you come in

What should I bring?

Your insurance card and ID, a list of your medications, and any results you have: PSA or urine tests, imaging reports, and pathology reports. If your scans are on a disc, bring the disc so Dr. Su can review the images with you.

Will my insurance cover the visit?

Long Beach, Koreatown and Santa Monica are in network with Medicare, CENTER IPA, and PPO plans from Aetna, Anthem Blue Cross, Cigna and UnitedHealthcare. Beverly Grove is in network with Medicare and most PPO plans. Call the office to confirm your plan. More on insurance.

Do I need a referral?

That depends on your plan. HMO plans usually need one; many PPO plans and Original Medicare do not. The office can check before your visit.

How do I cancel or reschedule?

Please call the office at least 24 hours ahead so another patient can have the time.

What if my symptoms are urgent?

Don’t wait for an appointment. Fever with a kidney stone, not being able to urinate, sudden severe pain in a testicle, or heavy bleeding with clots need care right away. Call 911 or go to the nearest emergency room.
