What you send us
When you use the request form, you send us your name, phone number, email, the office you chose, the reason for your visit, your insurance plan and anything you type in the message box. We use it only to contact you, schedule your visit and check your coverage.
Online requests are delivered to the practice through Hero EMR, our electronic health record and scheduling system, which protects them as health information under HIPAA. If the form opens your email app instead, or you choose to email us, that message travels like any other email, so please keep it brief.
What we don’t do
- We don’t sell or share your information for marketing.
- We don’t use advertising trackers or cross-site cookies on this website.
- We don’t ask for, and you shouldn’t send, Social Security numbers or payment details through the website.
Other services this site uses
Fonts are loaded from Google Fonts, and the site is delivered through Cloudflare. Like any web server, they receive your IP address and browser details to deliver the page. Online booking, when available, runs inside a secure Hero EMR window.
